Strategy Grant Reporting Requirements

If you received a Strategy Grant from the Foundation, you are required to complete a Strategy Grant Report.

We are interested in the impact of your work and how our grant contributed to your efforts. Begin with the award letter that informed you of the Foundation's decision to provide a grant to your organization. Please address the specific questions noted in the grant award letter when completing your report or in a separate attachment.

The evaluation process is designed to:

  • Provide an overview of the status and impact of your project by documenting what happened and why
  • Describe the lessons learned in the process of designing and implementing your project
  • Provide the Foundation with information on the positive changes in your community
  • Inform the Foundation's future grantmaking for similar projects or organizations attempting to replicate your efforts

Deadlines

The deadline for your evaluation report is indicated in the Foundation's grant award letter. In most cases, final reports are due 13 months after a grant was awarded. Your organization will not receive subsequent Foundation funding unless you have submitted all necessary evaluation reports.
