Grant Report Forms

If you have received a grant from the Foundation, we are interested in learning about the impact of your work. This is one of the most important ways the Foundation can learn about the effect our investments make throughout the state. 

If you are seeking additional funding from the Foundation, you are required to bring us "up to speed" with reports on all grants.  If a project is complete, please file the relevant report form from the list below.  If the project is not yet complete, you must complete an interim report.  The Strategy Grant and Newport County Fund grant programs provide a separate form for an interim report (see below).  For all other programs, please report on your progress (and spending against the grant) using the form below for the type of grant you received.
